New Patient Health Questionnaire

To provide the safest and most effective care, please complete your New Patient Health Questionnaire before your appointment.

You can either email it to [email address] or print and bring it with you to your visit. Completing this ahead of time allows Dr. Adler and the team to prepare a treatment plan tailored to your needs.

PRE + POST PREP

Umbilical / Mesenchymal Stem Cell Infusions

    • Hydrate well: Drink 20–30 oz (600–900 mL) of water 1–2 hours before your appointment to stabilize blood pressure and prevent dehydration headaches.

    • Eat lightly: Have a small, balanced meal (protein + complex carbs) to keep blood sugar steady. Avoid fasting, as it can worsen vascular sensitivity.

    • Avoid caffeine and alcohol: For 24 hours before and after your infusion, as both can alter vascular tone.

    • Ask about pre-medications: Some clinics offer a low-dose, non-sedating antihistamine (like loratadine) or acetaminophen to reduce minor inflammatory reactions. Only take under provider guidance.

    • Infuse slowly: 30–60 mL of product should typically take 30–60 minutes. Rapid infusion can trigger vascular dilation and headaches.

    • Maintain a comfortable environment: Dim lighting, calm breathing, and staying still can help prevent dizziness or pressure changes.

    • Hydrate again: Drink another 20–30 oz of water over the next 2–3 hours.

    • Rest: Allow your immune and repair pathways to respond naturally. Avoid strenuous activity or heat exposure that day.

    • Optional: Magnesium-rich foods (e.g., spinach, almonds) may help reduce vascular tension.

Exosome-Only Infusions

    • Hydrate generously: Follow the same 20–30 oz guideline.

    • Maintain stable blood sugar: Eat a light snack 30–60 minutes before infusion.

    • Avoid certain supplements: Do not take supplements that increase nitric oxide or circulation (like arginine, beetroot, or high-dose niacin) that day, as they can increase vascular sensitivity.

    • Request a slow drip rate: Exosomes should ideally run over 45–90 minutes, especially if concentrated.

    • Monitor for discomfort: If you feel rising headache or facial flushing, notify the nurse so the infusion rate can be adjusted.

    • Hydrate and move lightly: Walking helps circulation rebalance.

    • Rest well: Sleep early; cellular signaling and recovery processes are most active during rest.

    • Avoid alcohol and high-sodium foods: For 24 hours to limit vascular stress.

FAQS

Stem Cell & Exosome Infusions:

    • Umbilical/Mesenchymal infusions: typically 30–60 minutes for 30–60 mL.

    • Exosome-only infusions: ideally 45–90 minutes, especially if concentrated.

    • Stay comfortable, in a calm environment.

    • Let the nurse know if you feel dizziness, pressure, headache, or facial flushing.

  • Electrolyte drinks (coconut water or low-sugar rehydration solutions) within a few hours can significantly reduce the chance of headaches.

  • Stem cells are specialized cells in your body that can develop into various types of cells depending on your body’s needs. Think of stem cells as nature’s repair team—they travel to damaged areas in your body and transform into the specific cells your body needs, whether muscle, cartilage, skin, or even nerves.

Stem cells are remarkable for their ability to self-renew. They produce more stem cells, creating a continuous supply to help your body repair itself naturally. This ability to transform into specific cells makes them an ideal option for treating injuries, diseases, and degenerative conditions such as arthritis or nerve damage.

For further information on the properties of stem cells, refer to PubMed’s comprehensive database: PubMed – Stem Cell Research

  • When you receive stem cell therapy, the stem cells travel to damaged areas in your body and transform into the cell type needed, such as bone, muscle, or cartilage cells. This process helps repair and regenerate damaged tissue.

Stem cells also send healing signals to surrounding cells, which help reduce inflammation, prevent cell death, and stimulate growth and healing of other cells. Their ability to influence the surrounding environment makes stem cells a potent therapeutic tool for conditions such as joint pain, sports injuries, nerve damage, and organ repair.

  • Cord blood, collected from the umbilical cord after birth, is a rich source of stem cells that are younger and more effective than adult stem cells. These younger cells are better at regenerating tissue and repairing damage.

The collection process is completely non-invasive and safe for both mother and baby. Cord blood-derived stem cells also have a lower risk of rejection, as they have not been exposed to environmental stressors. This makes them an excellent option for patients needing regenerative treatment and immune modulation.

For studies on cord blood-derived MSCs, see research articles on PubMed: Cord Blood Stem Cells

  • Stem cell therapy can support recovery from a wide range of conditions:

    • Joint Pain & Arthritis: Regenerates damaged cartilage and improves mobility.

    • Muscle & Tendon Injuries: Regenerates tissue and accelerates healing.

    • Nerve Damage: Repairs nerve cells and restores function after stroke, spinal injury, or neurodegenerative disease.

    • Heart Disease: Regenerates heart tissue after a heart attack and improves heart function.

    • Lung Diseases (COPD, Fibrosis): Repairs lung tissue and improves breathing.

    • Chronic Wounds, Scars & Burns: Promotes faster healing and reduces scarring.

For a comprehensive guide on patient outcomes, see PubMed: Stem Cell Therapy for Joint Pain and Inflammation

  • Stem cells from umbilical cord blood are collected safely after a baby is born. The process is simple, painless, and does not harm the mother or baby. These stem cells are then cleaned, stored, and grown in a lab to be used later for treating injuries, arthritis, or nerve damage.

  • Stem cells do not carry donor DNA permanently, nor do they transmit vaccine proteins. Vaccines are broken down by the body within days, so they are not present in the stem cells you receive.

  • Research indicates that stem cells obtained from healthy sources and thoroughly tested do not cause cancer. They are used safely in many therapies. Your doctor will review your medical history to ensure the treatment is safe, especially if you have a history of cancer.

  • Unlike traditional treatments that mask symptoms, stem cell therapy targets the underlying cause of your condition. Whether for joint pain, muscle damage, or nerve injury, stem cells offer a natural healing solution that regenerates tissue and promotes long-term recovery without invasive surgery or medications.
